Yaesu FT2DR memory banks problem

Yaesu FT2D was programmed (without chirp) to have banks for several locations. chirp can read from the radio fine, but any access to the banks causes a modal error dialog with "Failed to add XX to mapping: list index out of range", where XX is the Loc being accessed. (screen shot included.) At least one Loc does show (correctly) as being assigned to a bank.

All errors and validation occurred on MacOS.
chirp version is daily-20170714

[ft1d] Prevent failure when radio-preset entry is in banks. Fixes #5167

Error was first described in Yaesu FT2D and exists in FT-1D.
Radios can be manually configured to use preset frequencies.
CHIRP will now ignore such entries; CHIRP cannot change but will LOG.warn them.
