Have you been able to resolve this on your own since you submitted this?

This looks like a cable issue. Please refer to these Wiki articles:
CableGuide
CableGuide FTDI OEM Cables
[RTSystemsCablesAndMavericks]

Windows: If you are using a generic cable with a Prolific chip, you will very likely need to downgrade your driver to version 3.2.0.0.
It can be found at http://www.miklor.com/COM/UV_Drivers.php
MacOS: this OS is apparently very picky about USB to Serial cables. From what I heard, only (genuine) FTDI-based cables can be made work.
You must have the KK7DS Python runtime for Mac OSX installed.

I know this is an old report, but would like to add my 2 cents, if possible to reopen it.
Scenario:

2 identical (it seems) Yaesu FT-7900.

Steps to reproduce this error using CHIRP next-20230129 (but it happen also with older version) :

1) Upload a file into radio #1
2) Download the file from radio #1
3) Try to upload the file to radio #2 (same envirnment, same cable used in step 1 and 2)
4) You get error "Radio did not ack block at 0"

Should you need any additional information, test or file, just ask.
Ciao.
